Dermatoscope Skin Microscopes for Dermascopy

The Dermatoscope is a diagnostic tool, a magnifier (typically 10x) with light source to examine skin, hair, tumors, cutaneous pathology. It is a skin surface microscope with diffuse illumination. Details in surface tissue color, borders, structures can be more critically seen and documented. Instrument illumination and oil (I used mineral oil) brings out features not seen with loupe binocular magnification and is much more portable than the operating microscope. The type Illumination source greatly changes what is seen. Traditional instruments were non-polarized and need oil or liquid medium between the glass plate of the instrument and skin to inspect skin lesions. Polarized light sources cancel out most skin surface reflections and permit exams without the oil. Images or videos can be captured and digitally processing them for records and or patient education "digital epiluminescence dermatoscope". Product choice depends on portability desired, wire connections tether the unit. Skin and pathology appearance varies with light source, polarization, oil, pressure, and sometimes temperature of the room. Find Your Dermatoscope.

Why All the Confusing Terminology?

Epiluminescence microscopy, dermatoscopy, dermoscopy, surface microscopy, direct skin microscopy, incident light microscopy and skin microscopy are interchangeable phrases used in this field. Each manufacturer plays the marketing game their own term try to differentiate themselves. The result is a confusing set of terms that really call for a standardization.

What Immersion Oil?

I preferred simple mineral oil applied with a cotton tipped applicator to each area of skin examined. The patient was then given tissue to clean up after the exam. Others like alcohol. I cleaned my Dermatoscope with an Alcohol wipe after each use. Polarized Light device advantages were designed to get away from needing the fluid interface. Yet, some features just need that liquid to be seen well. How much one is trading convenience of exam vs. diagnostic ability has not been well defined. There are devices that offer either option, permitting the exam to be optimized. However, for monitoring changing lesions, one needs to keep the light source / immersion methods consistent.

What are the limitations of dermoscopy?

While surface skin evaluation can supplement a clinical exam, you are only looking down to the level of the papillary dermis. Pathology in the reticular dermis are not seen. Amelanotic melanoma is missed with this method and heavily pigmented lesions can obscure scoring structures requiring experienced skills for clinical judgment. If in doubt, biopsy. A negative surface exam is no guarantee that there is no pathology.

How much experience needed?
The dermatoscope is just a tool like a set of magnifying loupes or operating microscope. Use requires experience, clinical judgment, and skills. Each type instrument has its own characteristics and nature. There is a learning curve.
Dermatoscope and the patient.

There was a major benefit for the returning patient and a lesion can be compared to prior documentation for any changes. I was also able to tell my patients let us watch this one and that one, biopsy this one, and I saw nothing of concern for that one. During my exam of each lesion of concerned to the patient, about helped with my patient education programs about Sun Protection, skin cancer risks, monitoring their skin, and returning to see us for any changes. For patients with history warranting closer follow up, the prior score for any watched lesion.

The a digital camera brings new potential for documentation and the medical record.

Scoring Method for Diagnosis Malignant Lesion

Major ELM Criteria Score
Atypical pigment network 2
Gray-blue areas 2
Atypical vascular pattern 2
Minor ELM Criteria Score
Radial streaming (streaks) 1
Irregular diffuse pigmentation (blotches) 1
Irregular dots and globules 1
Regression pattern 1

The score for criterion presence is determined on the basis of the odds ratio. Odds ratios measure the capacity of each criterion of increasing the probability of a positive melanoma diagnosis. By simple addition of the criteria scores, a minimum total score of 3 is required for the diagnosis of melanoma.

Bermant Pigmented Lesion Exam

The unit I bought had a ruler permitting measurements. For pigmented lesions, I built a sheet to score each suspicious lesion as part of my analysis permitting tracking lesions for my Reconstructive Plastic Surgery patients.

3Gen DermLite III DL3 Polarized & Fluid Dermascope

3gen dermlite III DL3 polarized dermascope

The DL3 has been developed to be the tool of choice for the world's leading dermatologists. Precision-engineered and crafted from solid aluminum, it is the first hand held DermLite to integrate a 25mm four-element lens, which offers greatly reduced optical distortion and a sharper image across the field of view. Twenty-one high-powered LEDs produce approximately 30% more illumination in cross-polarized mode than the DermLite II HR, and seven non-polarized LEDs for immersion fluid dermoscopy create a brighter image than the equivalent mode in the DermLite II hybrid. For optimum ease of use, one button does it all: A quick tap instantly toggles between polarization modes, and a longer push turns the unit on or off.

Buy the 3Gen DermLite III DL3 Polarized & Fluid Dermascope.
  • Up to 2500 foot-candles of light (at faceplate)
  • Polarized or non-polarized
  • Retractable, fully removable faceplate with 10mm scale
  • Four-element 25mm 10x lens
  • 28 high-powered light-emitting diodes (LED)

A major step forward is the DL3's fully retractable faceplate spacer design and focusing mechanism. By twisting a large precision dial that is as easy to turn when mounted on a camera as it is during regular use, you extend its faceplate spacer and focus your image with an available focal range in excess of ±4mm. The innovative spacer comes with a glass faceplate with 10mm scale, and as part of a late 2012 engineering revision that is now shipping, the new design eliminates any leakage of immersion fluids into the interior. Now even more than ever, contaminants as well as undesirable ambient lighting no longer affect your image.

Of course, the DL3 is camera compatible. Attach your digital camera* via the product's built-in threaded 28mm connection, extend the spacer, and start taking sharp, glare-free images quickly and easily. The same 28mm connection may be used to mount the included eyepiece, which may greatly improve the contrast of the image and helps reduce internal reflections caused by ambient lighting.

DL3 is powered by the same integrated, rechargeable lithium-ion battery used in the DermLite II series, which can be recharged via the included charging base. A DermLite DL3 will let you know when it's time for a recharge: While using a fully charged DL3, the "3" on the top of the unit lights up green; once the charge level drops below 25%, the "3" will glow orange.

DermLite DL3 combines the unique advantages of polarized and immersion fluid dermoscopy in one truly refined device that offers superior illumination and optics, solid durability, and, with its ergonomic handle design and single button operation, a degree of convenience that is unsurpassed in a DermLite.

Each unit comes with 3Gen's 5-year warranty, includes a leather belt case, lithium battery and charging base, eyepiece, and lanyard.
